Diabetes Care Program

The Diabetes Center is available to all people with Diabetes: Type 1, Type 2, gestational, or pre-diabetes. Programs are designed to meet the needs of the newly diagnosed person or those who have had Diabetes for many years.  Individual or group instruction is available. Specialized programs focus on individual needs and may include education, monitoring, medication review, coping skills, weight loss, exercise, and nutrition.

The Diabetes Center is recognized by the American Diabetes Association and is reimbursed by Medicare.  Our program is plan specific for other insurers.  Patients are referred by their physician.

Our "Learning to Live with Diabetes" program is a complete education series in all areas of Diabetes self-management.  The series includes 10 hours of education and ongoing follow up. 

Supportive programs include:

  • Starting Insulin
  • Medical Nutrition Therapy
  • Insulin Pump Therapy                                                        
  • Continous Glucose Monitoring
  • Children and Diabetes Program
  • Maintenance:  Exercise and Nutrition Programs
  • Diabetes and Pregnancy Program
  • Resource library and internet access available at the Center
