VCC Social Services Department The Social Service Department at the Virginia Convalescence Center is here to serve you and to help make your admission to this new setting both smooth and comfortable. We make every effort to learn as much as we can during the admissions process in order to meed your needs. Social Services: Meets with new residents and their families upon admission to review admission information. Offers assistance and support with any possible adjustment issues. Offers ongoing discharge planning, providing information regarding local assisted living facilities, home care agencies, adult foster care, and memory care units. Assists residents with meeting any emotional needs. Provides informaiton and assistance with completing a Health Care Directive/Living Will. Provides information regarding completing a Power of Attorney document and Guardianship. Provides information regarding Medicare, Medical Assistance, and private insurance. Provides information regarding Hospice care, grief counseling, and grief support groups in our area.
